Investigation Summary
At 8:08 a.m. on April 21, 2013, an employee who was a painter, was left working alone. He was later found dead, lying face-down on the floor. Prior to the discovery, the employee was using an airless sprayer to apply wood finishes to cabinetry, interior doors and moulding. The wood finish products included but were not limited to stain, lacquer thinner, lacquer finish and lacquer sealer, each of which contained volatile organic compounds (VOC's). The windows and doors to the room in which the employee was working were closed and sealed in sheets of plastic. The ductwork in the room for the building's general ventilation system was sealed with painter's plastic. No other means of mechanical ventilation was provided. The employee was issued a negative pressure, air-purifying half-mask respirator equipped with organic vapor cartridges; however, when his body was found, he was not wearing it. The employee died as a result of the inhalation of solvent (lacquer retarder) fumes.
