Employee Is Injured in Fall from Ladder Jack Scaffold

Employee #1 was working from a cantilevered ladder jack scaffold. The scaffold became displaced, and he fell. The investigation revealed that the scaffold plank was attached to the bracket on a ladder at one end, but was resting on a fire escape rail on the other. The employee sustained unspecified injuries in the incident.
