Investigation Summary

On June 26, 2007, Employee #1 was operating a Wirtgen Model Number WR 2500 Reclaimer-Stabilizer along a section of dirt road. The area had received a coating of lime slurry and the approximately 60,000-pound vehicle was using a centrally located, spiked cylinder to mix the soil and lime slurry. Employee #1 drove the vehicle, and it went over an approximately 45-degree embankment, rolled over, and came to rest on its left side. Employee #1 suffered blunt force trauma to his face and head and was declared dead at the scene. The driver's cab was air conditioned, but it had no effective roll-over protective structure designed as part of the vehicle. It was also not equipped with a seat belt.
