Investigation Summary
At 2:22 p.m. on May 25, 2023, Employee #1 and a coworker were installing metal cap paneling on a wastewater treatment tank. As they were facing each other near the roof peak, standing on the roof braces beneath the panel, Employee #1 side-stepped down the panel. The far edge broke loose and fell inwards, and the panel beneath Employee #1' fell to the bottom of the pit. The roof panel remained attached on the side Employee #1 was standing and acted like a ramp for the employee to slide down on and into the pit. Employee #1 was wearing a harness attached to a safety rope. A coworker indicated the "rope catch" did not work properly and failed to stop Employee #1 descent. The employee was killed due to multiple traumatic injuries.
