Investigation Summary
At 4:12 p.m. on October 25, 2022, Employee #1 was trying to connect two peanut wagons together. The tractor operator stepped out of the tractor while it was running to assist Employee #1 and a coworker connect the empty peanut wagons. The tractor reversed from where the operator left it and struck Employee #1 and pinned him to the wagon. Employee #1 was killed.
