Investigation Summary
At 12:00 a.m. on November 19, 2021, a trailer loaded with merchandise was being picked up for transport by a third-party company. The trailer was parked next to the building at a loading dock. Employee #1 was tasked with removing the safety locks on the trailer hook points prior to departure. Employee #1 walked along the passenger seat side of the tractor and made signs of a stop to the transport driver so he would have time to remove the locks. The transport driver never stopped. As the tractor was moving backwards to attempt to connect to the trailer, Employee #1 began walking to the front of the trailer. Employee #1 was crushed between the front of the trailer and the back fifth wheel of the tractor. Emergency services were called, and Employee #1 was transported by EMS to the Atrium CMC Main hospital. Employee #1 was killed. The CMPD report listed the impression of death as "traumatic injury."
