Investigation Summary
On August 23, 2021, an employee working for a property management company was operating a propane powered T20 Tennant floor scrubbing machine (Serial Number T20 -10131) to clean a 80,000 square foot warehouse. The warehouse was vacant, and the company was contracted to clean the warehouse before it was leased. At 10:00 a.m., the employee began working alone. A coworker, the project manager, was communicating with the employee by phone and text. At 6:00 p.m., the coworker noticed the employee was not replying to texts or calls. At 8:30 p.m., the coworker drove to the site. At 8:56 p.m., the coworker found the employee unconscious and slumped over while in the driver's seat of the scrubber. The coworker called emergency services which transported the employee to the hospital. The employee was admitted, and toxicology reports confirmed an overdose of drugs. The employee was hospitalized to treat hypoxemic respiratory failure, rhabdomyolysis and substance abuse, specifically, ingestion of cocaine and cannabis.
