Investigation Summary
At 6:15 a.m. on August 2, 2021, Employee #1, a forklift operator at an empty can management facility, was staging pallets of cans for shipment. While traveling with the forks raised to move pallets from high stacks, he swerved to avoid a structural pole. The forklift began to tip, and as he attempted to jump free, the overhead cage caught him and crushed him between the cage and the warehouse floor. He sustained crushing injuries to the abdomen and was transported to Modesto Memorial Center, where he was admitted for treatment.
