Investigation Summary
At 5:45 p.m. on August 4, 2021, an employee was assembling a three-seat sofa on the north side of Cell 58. He had just stapled components into place when, while holding the staple gun in his non-dominant left hand, he inadvertently triggered it. The staple gun unexpectedly contacted his forehead, discharging a 16-gauge, galvanized chisel point staple that fully penetrated his forehead. The employee was hospitalized for treatment of the head injury.
