Investigation Summary
At 10:15 a.m. on July 16, 2021, Employee #1, along with two other employees and the Plant Supervisor, had been working in a single-entry pit under the Hunter 20 Mold Loading machine to remove sand mold chunks and spilled iron because the turntable would not turn. After exiting the pit, Employee #1 reentered for an unknown reason despite being instructed not to. The machine's Lockout/Tagout had been removed, and the machine was cycled. When the mold table cycled a second time, Employee #1 was struck in the back by the lowering bucket, becoming trapped between the bucket, the grate he was sitting on, and a piece of iron supporting his head. Employee #1 sustained injuries to the upper body and back and lost consciousness. The employee was hospitalized.
