Investigation Summary
At 2:45 a.m. on May 27, 2021, an employee, a napkin operator, was manually refeeding tissue into the napkin press and folder machine. The employee jogged the machine, unaware that her left hand was located near the pinch point between two rollers. The employee's finger was pulled into the machine's rollers and she suffered a partial amputation of her left index finger. The employee was hospitalized.
