Investigation Summary
At 1:30 p.m. on April 7, 2021, Employee #1 was operating a forklift when he heard an emergency siren in building #3, indicating a problem with the baler. He attempted to reset the wires on the drive side of the machine (control panel side) when the tie cycle of the baler was activated. As Employee #1 was pushing on the wire, his right hand became caught and was pushed by the force of the mechanism. He sustained a severe laceration to his right hand and was hospitalized for treatment.
