Investigation Summary
At 2:20 a.m. on March 23, 2021, Employee #1 was replacing a motor on a gearbox. After realizing the motor did not fit, he used sandpaper to sand down the motor end for proper fitting. Employee #1 asked a coworker to pump the motor while he continued sanding. During this process, the glove on Employee #1's left hand became caught in the motor shaft, resulting in the amputation of his left index finger. Employee #1 was treated without hospitalization.
