Investigation Summary
At 2:30 p.m. on January 27, 2021, Employee #1 was replacing the hydraulic reservoir cover on a bin dumper at a waste management facility. While working near the dumper, the operator started the dumper, causing it to move unexpectedly. The unexpected movement of the dumper struck Employee #1, resulting in the employee sustaining one broken left rib and four broken right ribs. Employee #1 was transported to the hospital for admission and treatment.
