Investigation Summary
At 7:40 a.m. on January 15, 2021, Employee #1, employed at a manufacturing facility, was inspecting the hopper on line J, an injection molding machine, which had been disassembled by maintenance technicians during the third shift. As material started coming out of the disassembled hopper, the employee attempted to prevent the material from falling by placing his hand near the opening at the bottom of the hopper. Employee #1 believes the agitator inside the hopper caught his glove, pulling his finger into the machine. As a result, the employee sustained an amputation of the middle finger on his right hand. The employee was treated without hospitalization for the injury.
