Investigation Summary
At approximately 11:13 a.m. on September 15, 2020, Employee #1 was pulling up a 2x6 fascia board to his working level when he lost his balance and fell. The fall resulted in a torn shoulder ligament, bruised ribs, a bruised lung, and a concussion. The employee fell to the ground upon impact and was hospitalized for treatment of the injuries.
