Investigation Summary
At 1:00 a.m. on October 5, 2020, an employee was feeding a sheet into the conveyor. The sheet buckled on his side of the conveyor. The employee had already placed the conveyor in "run" mode, and when he attempted to pull the sheet down, the conveyor caught the sheet and pulled his hand into the conveyor. The employee suffered a left-hand index finger amputation, a middle finger fracture, and a ring finger laceration. The employee was hospitalized.
