At 8:00 p.m. on October 13, 2020, Employee #1 was watching the maintenance leader weld a broken metal piece on the block making machine. Employee #1 and several coworkers became ill due to exposure to carbon monoxide and were transported to a local hospital. Employee #1 suffered a high carboxyhemoglobin level, resulting in headache and mild delirium. He was hospitalized.
