Investigation Summary

At 9:15 a.m. on May 6, 2020, Employee #1 was using a pallet wrap machine to wrap a pallet. As Employee #1 was adjusting the stretch wrap to cover the bottom of the pallet, the tip of Employee #1's left middle finger got caught between the moving pallet stretch wrap machines rotating platform and the base frame of the stretch wrap machine. Upon catching the tip of her finger, Employee #1 pulled her finger away from the pinch point. As Employee #1 pulled her finger away from the pinch point, she noticed that the tip of her left middle finger was severed, and she contacted her supervisor. Employee #1's supervisor transported her to Gardenia Memorial Hospital, where she was temporarily treated. Employee #1 was transferred to Western Hand and Orthopedic Specialist in Gardena, where surgery was conducted to repair the finger. The surgery included the amputation of the tip Employee #1's left middle finger. Employee #1 was hospitalized.
