Investigation Summary
At 10:50 a.m. on March 12, 2020, Employee #1 was employed by a construction electrical contractor. Employee #1 was standing on the 4th step of a 9-foot aluminum A frame ladder, while working on an electrical component above the drop ceiling. Employee # 1 fell from the ladder suffering a fractured clavicle, fractured ribs and fractured vertebrae. Employee # 1 was hospitalized. Employee #1 had no recollection of the fall and no reason was discovered during the investigation.
