Investigation Summary
At 12:00 a.m. on February 28, 2020, an employee was making adjustments to a grating that was going to be welded to a thruster. The employee climbed on a forklift to reposition the grating and accidentally activated the lever to move the mast toward him. The employee suffered broken ribs and arms, when the mast and the frame of the power industrial truck crushed him. The employee was hospitalized.
