Investigation Summary
At 3:00 p.m. on February 14, 2020, an employee in her mid-fifties was working for a nationwide chain of mini-warehouses and self-storage facilities. She was walking outside to assist a customer, when she slipped and fell on ice. She suffered three fractures to her femur. She was hospitalized. Her employer did not report the inpatient hospitalization that was the result of a work-related incident to OSHA within twenty-four hours. The injury was reported to OSHA on February 17, 2020.
