Investigation Summary
At 8:00 a.m. on January 7, 2020, an employee was trying to cut out an air bubble from a sheet of 1/4 inch plastic that was being pulled by the pull roller on an extruder. The employee was using a utility knife to cut the air bubble when the glove on his right hand got caught and pulled into the pull rollers. The unguarded pull rollers are 2000 pounds and pull large sheets of plastic from the extruder. The employee was hospitalized to treat crushing injuries to his right hand and arm.
