Investigation Summary
At 8:10 a.m. on November 22, 2019, an employee, a baker, was working in the baking room of the restaurant. He was tasked to make a new batch of dough for dinner rolls to be sold the following day. The employee was operating a Hobart model H600T commercial vertical mixer, and had loaded the ingredients into and turned on the 60-quart mixer with a spiral hook dough arm agitator attachment, to begin mixing in second speed while he attended to other activities. When the employee returned to the mixer, as it was nearing completion, he pressed the stop button to stop the mixer. Prior to the agitator stopping motion, the employee stated that he opened the top guarding with an interlock, and attempted to pull out the mixing bowl with his right hand. The dough arm's residual motion struck his right thumb, causing an amputation below the nailbed. The employee was taken to Mercy Folsom Hospital, where he was then transported to San Francisco General. The employee was hospitalized and released on November 25, 2019.
