Investigation Summary
At 4:45 a.m. on November 9, 2019, an employee was cleaning the rollers on a machine when he bypassed safety mechanism by blocking the safety sensor with a piece of metal. The employee used the foot pedal after tricking machine to think it was in clean mode and got his hand caught in the rollers of the machine. Employee was hospitalized to treat amputations of two fingers.
