Investigation Summary
At 11:15 a.m. on October 8, 2019, an employee, a maintenance worker, was changing a ballast in a light fixture in a grocery store. The employee used a ladder to gain access to light fixtures located in the backroom of the store and discovered that the lights had bad ballast. The employee disconnected the bad ballast and was installing a new ballast into the fixture when he was shocked due to not deenergizing the store's 277 VAC electrical system. The employee was thrown from the ladder and was hospitalized for treatment of his injuries.
