Investigation Summary
At 6:45 a.m. on August 22, 2019, an employee and coworker attempted to remove a four-foot-by-eight-foot mirror glued to a bathroom wall. While the two worked to remove the plywood backing on the mirror, the mirror shattered, and the employee received a severe laceration to the right forearm. The employee was taken to the hospital and admitted to treat the injury.
