Investigation Summary
At 9:15 a.m. on August 14, 2019, an employee was working in front of the Trim Press area of a thermoforming machine packaging plastic lids. As the lids exited the machine, the employee reached into the machine, which was not properly guarded. As the employee attempted to remove the lids he was reaching for, his right hand was struck by the cutting blade. The employee amputated the distal tip of his middle finger, requiring medical treatment without hospitalization.
