Investigation Summary
At 9:30 a.m. on July 22, 2019, an employee worked in a trench box in an excavation to direct an excavator operator to where to dig out dirt to find the end of a pipe to connect new sections of pipe. At some point, the employee was struck by the excavator buck and pinned to the trench box spreader. The employee was hospitalized to treat multiple fractures.
