Investigation Summary

At 10:45 p.m. on July 27, 2019, a middle-aged employee was working as a millwright for a structural steel and precast concrete contractor. He was performing post-decking detail work during an alteration or rehabilitation project at a manufacturing plant. He and a coworker were installing upgraded safety guardrails to prevent employee contact with moving parts of a conveyor system. They were drilling holes in the concrete floor to set anchors into for bolts to attach the guardrail system. The employee stood up, stood still for a moment, uttered the word "whoa," and then lost consciousness and fell over. He had suffered a heart attack. He died. This report was filed in error. The fatality was not covered by OSHA.
