Investigation Summary
At approximately 3:15 a.m. on May 3, 2019, an employee was adjusting a bolting machine that failed to fully cycle. When the employee made the adjustment, the employee's finger was caught between the slides and the frame of the machine. The employee suffered a partial amputation of the tip of the finger metal slide and frame of the machine. The employee is hospitalized for these injuries.
