Investigation Summary
At approximately 10:00 a.m. on May 9, 2019, an employee was attempting to manually push a steel coil onto a press loading mandrel to align it. The mandrel arm/coil arm slid off the top of the coil and pinched the employee's finger between the arm and the upper part of the steel coil. There was an amputation of the right index fingertip, and the employee was hospitalized for these injuries.
