Investigation Summary
At 7:15 a.m. on January 30, 2019, an employee was drifting a 40 pound, 3 foot by 5 inch lead sleeve, onto a wooden dowel. The employee was striking the wooden dowel/lead sleeve set to slide it onto a wooden log. The employee caught his left hand in the lead sleeve during the process and incurred a laceration, damaged tendons and a fracture of the pinky finger, requiring hospitalization.
