Employee falls headfirst into sump tank and asphyxiates

At 1:30 p.m. on March 12, 2019, an employee was taking the water out of a 4 foot x 4 foot underground sump tank at a gas station when he fell head first into the tank. The employee was killed from positional asphyxiation.
