Investigation Summary
At 11:45 p.m. on January 31, 2019, an employee worked as a rigger/signalman and attached rigging gear to steel joints. The employee then signaled the crane operator to lift the load. The load was raised and the employee became entangled in the tagline and lifted into the air. At some point, the employee was able to get untangled and fell eight feet. The employee received a fractured pelvis and was hospitalized for treatment.
