Investigation Summary
At 8:45 a.m. on November 16, 2018, an employee was attempting to retrieve a television from the upper rack of stock room. The employee failed to utilize a fall protection lanyard and body belt of cherry picker. He threw the lanyard and belt over the rear of the machine. At some point during ascent or initial descent the lanyard got stuck in the chain of the cherry picker that controlled the platform of the machine. The employee utilized a mallet and knocked the lanyard lose which resulted in the machine's platform falling approximately 8 to 10 feet. The employee broke his right ankle during the fall after contact with the concrete ground.
