Employee incurs multiple lacerations when struck by Gang Rip

At 4:55 p.m. on November 8, 2018, Employee #1 attempted to install the arbor and blade assembly to the spindle for the Gang Rip Saw. While installing the assembly, the employee sustained a laceration to the left forearm, right palm, and pinky finger. He did not apply lockout/ tagout. The employee was hospitalized.
