Investigation Summary
At 11:30 a.m. on October 11, 2018, an employee was walking across a bridge deck on top of concrete panels that were just placed on their foam strips. As the employee walked across the deck panels (measured at four foot wide by four and a half feet long), they shifted and fell out from under the employee. The employee and deck panels fell approximately sixteen feet where the employee landed on top of the deck panels. The employee fractured his back and ribs and was hospitalized.
