Investigation Summary
At 1:00 p.m. on August 30, 2018, Employee #1 was aiding a production coworker with tasks requiring full body entry into a large stamping/press machine. The interlock door system was opened with the normal controls to allow Employee #1 to enter the machine. The normal controls were used to close the door system while Employee #1 was inside the machine. Employee #1 suffered partial amputation of the right leg. The employee was hospitalized.
