Investigation Summary
At 12:15 p.m. on April 20, 2018, Employee #1, employed by a heavy construction company, was working at the site of a two-story water/sewage treatment plant. He was setting a bulkhead when the bulkhead slipped. Employee #1's right hand was caught between the bulkhead and rebar. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for a severe laceration to his right index finger.
