Investigation Summary
At 1:30 a.m. on March 27, 2018, Employee #1 was operating a left mandrel winder that was located in the Spring Department of a facility. Operation of the winder involves an employee attaching a wire to the mandrel/bar and turning the machine on, which allows the mandrel bar to start rotating and wind the wire into a spring. The winder that Employee #1 was operating was not guarded. During work, Employee #1 became caught in the winder and his right arm was amputated. He was hospitalized and received treatment for his injury.
