Investigation Summary
At 6:00 p.m. on September 27, 2017, Employees #1, #2 and #3 were transferring LPG propane from a Bobtail truck tank to 1,000 gallon LPG tanks. There was a gas leakage. A forklift entered the area, causing an ignition of sparks and an explosion. Employees #1, #2 and #3 sustained second degree burns to fifty percent of their bodies. Employees #1 and #2 suffered complications from bronchopneumonia, and died.
