Investigation Summary
At 9:18 a.m. on November 9, 2017, Employee #1, a mechanic, was troubleshooting a malfunction of a preform dumping machine. The machine was deenergized. As Employee #1 used a metal pry bar to lift up the preform dumper bucket from the base of its frame and wedge a piece of wood between the two parts, the pry bar slipped and the bucket retracted. Employee #1's left thumb became caught between the frame and the bucket, resulting in its amputation. Employee #1 was hospitalized and received treatment.
