Investigation Summary
At 1:15 p.m. on April 5, 2017, Employee #1, employed by a telecommunications company, was free climbing a utility pole to make cable connections. As he approached the cable strand and went to take his last step, the wood under the stationary gaff split, the gaff cut out from the pole, and Employee #1 fell to the base of the pole, a fall height of 18 feet. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for fractures of his right leg.
