Investigation Summary
At 10:45 a.m. on June 7, 2017, an employee and a coworker were performing a tool change out on a thermoformer machine. Before work, the electrical energy and water supply were secured (lockout/tagout). However, the air supply was not. Nor was a safety jack installed, which the company implemented as part of their lockout/tagout program to provide added protection from crushing injuries during tool change outs. Without knowing that the employee's hands were still inside the machine, the coworker began to manually lower the machine's platen and crushed the employee's right hand.
