Investigation Summary
At 1:30 p.m. on June 21, 2015, an employee was conducting an inspection of a water tower and used a Miller body harness, attached to a Miller Manyard ll - two leg shock absorbing lanyard. The employee was between the 2nd rest platform and the top of the water tower when he fell, between 76 and 113 feet. The employee was wearing fall arrest protection equipment and it was attached to a ladder climbing safety trolley. The connection was made to provide a positioning device. The positioning device requires an additional fall arrest system to be used in conjunction, and the equipment in use was not modified to be sufficient strength to handle a person and tool weight greater than 310 lbs. The force of the fall, weight, and incompatible parts shredded the harness and the employee was found dead hanging with the harness tangled around his neck.
