EMPLOYEE DIES IN 40-FOOT FALL FROM STABBING BOARD

EMPLOYEE #1, AN OIL WELL CASER, WAS STABBING A 5 1/2-INCH CASING FROM AN A-FRAME MEMBER ABOVE THE STABBING BOARD. THE TRAVELING BLOCKS CAUGHT HIS SAFETY LANYARD AND BROKE IT, CAUSING HIM TO FALL 40 FEET TO THE RIG FLOOR BELOW. EMPLOYEE #1 DIED FROM THE RESULTING INJURIES.
