29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) Flatwoods Job Corps, 2803 Dungannon Rd, Coeburn, VA 24230: On February 27, 2017, at approximately 8:55 a.m., a Job Corps student suffered a fingertip amputation while using an ironworker. The incident resulted in an in-patient hospitalization at 1:00 p.m. the same day. The employer did not report the amputation or the in-patient hospitalization to the Occupational Safety & Health Administration until after the 24-hour reporting period. No abatement documentation or certification required.
