29 CFR 1904.39(a): 29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: (a). Workplace located at 9406 N. Highway 33, Osborn, Missouri- The employer failed to notify the Occupational Safety and Health Administration of a workplace fatality that occurred on July 28, 2013. OSHA was not notified of the incident until July 31, 2013.
