29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours. INTERNAL REVENUE SERVICE - CRIMINAL INVESTIGATION PHOENIX FIELD OFFICE: The agency did not provide copies of the injury and illness records such as OSHA 300 logs and 300A summaries to an authorized government representative within four business hours. The Internal Revenue Service was previously cited within OSHA's Region 2 for a violation of this occupational safety and health standard, which was contained in OSHA Inspection #1573774, Notice #2, Item #1, issued on July 06, 2022 with respect to a workplace located at City View Plaza, Guaynabo, Puerto Rico which became a final order on July 29, 2022.
